Why Mental Health Care Is Hard to Find in New York

New York's mental health landscape is uneven. Most specialists cluster in New York City and a handful of upstate cities, while residents across rural upstate New York often travel hours to reach a psychiatrist. Even in metro areas, waitlists for child and adolescent specialists can stretch for months.

More than 1 in 5 U.S. adults live with a mental illness each year, according to the National Institute of Mental Health. New York is no exception, yet many residents never see a specialist.

A 2025 New York State Comptroller report found that every rural county examined is designated a Mental Health Professional Shortage Area, with more than 305,000 residents classified as underserved across those counties alone.

SAMHSA data shows that roughly half of adults with a diagnosable mental health condition do not receive treatment in any given year, and the gap tends to be wider outside urban centers.

For many New Yorkers outside the city, the nearest psychiatrist may be hours away. Even in metropolitan areas, waitlists for child and adolescent specialists can stretch for months.
